Abstract
Objetivo: Describir las características clínicas, quirúrgicas y perioperatorias de una serie retrospectiva de pacientes con angiofibroma nasofaríngeo juvenil tratados mediante el Protocolo EMGS-MURILLO en un hospital público con recursos tecnológicos limitados. Materiales y métodos: Se realizó un estudio observacional, retrospectivo y descriptivo de 20 pacientes con diagnóstico histopatológico confirmado de angiofibroma nasofaríngeo juvenil, intervenidos en el Servicio de Cirugía Oral y Maxilofacial del Hospital Escuela, Tegucigalpa, Honduras, durante un período de cinco años. Todos los pacientes fueron tratados mediante el Protocolo EMGS-MURILLO, que integra ligadura de la arteria carótida externa ipsilateral e infiltración intratumoral de ácido tranexámico y peróxido de hidrógeno en proporción 1:1. Se incluyeron cuatro casos ilustrativos con registro fotográfico quirúrgico completo. Resultados: La edad media fue de 18,9 ± 3,9 años; 17/20 pacientes fueron del sexo masculino y 3/20 del sexo femenino. El abordaje Weber-Ferguson se empleó en 11/20 casos y otro abordaje transfacial en 9/20. El tiempo quirúrgico promedio fue de 116,3 ± 29,1 minutos y el sangrado intraoperatorio promedio fue de 138,8 ± 41,6 mL. Se documentaron complicaciones postoperatorias en 2/20 pacientes: una infección tardía y una exposición de material de osteosíntesis. No se registró necesidad de transfusión sanguínea ni recurrencia tumoral durante el seguimiento consignado. Conclusiones: El Protocolo EMGS-MURILLO mostró un perfil hemostático favorable y una baja frecuencia de complicaciones en el manejo quirúrgico abierto del angiofibroma nasofaríngeo juvenil.
